If you do not currently have an active relationship with BOLD Legal Defense and wish to pursue one, please complete this form and one of our representatives will contact you as soon as possible.
Upon acceptance, we will require a copy of your license, evidence of E&O (at a minimum of $1Mil/$1Mil limits with an acceptable carrier), completed W-9 Form, and a signed Agreement.
