How to Get Appointed with BOLD Legal Defense Insurance
Step 1: Complete the Required Documents
To get started, you need to fill out the two attached documents.
– Producer Agreement (Download).
– W-9 Form (Download).
Step 2: Submit Your Documents
You will need to submit the following:
– A completed and signed Producer Agreement.
– Copy of the declarations page of your current Errors & Omissions (E&O) insurance policy (we do not accept certificates of insurance).
– Copy of your current agency insurance license.
– A completed W-9 form.
